Keystone Point Of Service (KPOS) Program

To be eligible to participate in the flexible benefits plan, you must be age 30 or older with at least 5 years of service with a contributing food employer.

If you are eligible to participate in the UFCW Local 1776 and Participating Employers Flexible Benefits Plan, you may elect medical coverage under the Keystone Point of Service (KPOS) Program instead of the Personal Choice Program. You should refer to the Flexible Benefits Plan Summary as well as your collective bargaining agreement to determine whether you are eligible to participate in the UFCW Local 1776 and Participating Employers Flexible Benefits Plan

The KPOS Program offers flexibility and comprehensive health benefits. The KPOS Program is a combination of (a) HMO benefits, subject to care being directed through your primary care physician (PCP) (called referred benefits), and (b) coverage for services by providers of your choice without your PCP’s referral (called self-referred benefits). If you are covered under any other medical plan, check the section titled “Coordination of Benefits” to see how you should submit your medical claims.

Referred benefits under the KPOS Program are underwritten by Keystone Health Plan East, and self-referred benefits under the KPOS Program are underwritten or administered by QCC Ins. Co., subsidiaries of Independence Blue Cross - independent licensees of the Blue Cross and Blue Shield Association.

Independence Blue Cross offers products directly, through its subsidiaries Keystone Health Plan East and QCC Ins. Co. and with Pennsylvania Blue Shield - independent licensees of the Blue Cross and Blue Shield Association.


This section summarizes the current provisions of the KPOS Program. As a summary, it cannot contain all the details and provisions of the program. Upon enrollment, you will receive a Keystone Handbook (referred services) and a QCC Booklet-Certificate (self-referred services) which will contain more detailed information. The group contract purchased from Independence Blue Cross for the KPOS Program is incorporated by reference as if fully set forth in this booklet. If there are any differences between the benefit descriptions set forth in the group contract and the descriptions set forth in this booklet (or in the Keystone Handbook and QCC Booklet-Certificate), the group contract prevails. Healthy Lifestyles Program

You are also eligible to participate in certain programs that may help you maintain your health. As an Independence Blue Cross member, you can take advantage of a wide variety of innovative Healthy Lifestyles Programs. These programs provide you with incentives to help keep you and your family in the best of health.

Your participation in any of these Healthy Lifestyles Programs will not affect your benefits under the Fund's Physical Well-Being Program. You will still be entitled to benefits under the Physical Well-Being Program. You should refer to that section of the booklet for more details.

  Health Management - Weight Management with Weight Watchers, Fitness Reimbursement, Health Management Programs, and Smoking Cessation.
  Alternative Health - Alternative Health Discounts, Stress Management, and Better Sleep.
  Women's Health - Baby BluePrints Prenatal Program, Mammography Birthday Card Program, and Pap Test Screening Program.
  Family Health - American Red Cross Save-A-Life Course Discounts, Adolescent Health, Pediatric Immunizations, Child Safety, and Personal Health Profile.
