Changes In Coverage |

Plan coverage for you and your dependents continues
as long as you remain an active, full-time employee, in accordance with your
collective bargaining agreement. If you
become a part-time employee, you may be eligible for a different plan of
benefits under the Fund. If you
terminate your employment, retire or become disabled, your coverage under the
Plan ends. Your coverage will also end
if your participating employer does not pay the required contributions to the
Fund. Retiree Benefits When you retire, you may be entitled to some, but not all, of the benefits under the Plan. You must meet certain eligibility requirements to qualify. Generally, retiree benefits include: Please keep in mind that your retiree benefits may change from time to time, as determined by the Board of Trustees in its sole discretion. The cost to you, if any, for such coverage may also be changed at any time by the Board in its sole discretion. Personal Choice 100 and Mental, Nervous, Drug & Alcohol To be eligible for Personal Choice 100 and mental, nervous, drug & alcohol coverage for you and your dependents, you must have at least 10 years of full-time service with a contributing employer, be collecting a pension from a Food Industry Pension Fund and be between the ages of 62 and 65 at the time you retire. If you are receiving a Social Security Disability Pension, you are eligible regardless of your age. If you retire before age 62, you are not eligible for coverage immediately or when you reach age 62.If your spouse is over age 65 and is eligible for Medicare at the time of your retirement or becomes eligible at any time while you are eligible for retiree medical benefits, he or she will receive Personal Choice Supplemental coverage. When you reach age 65 or become eligible for Medicare, whichever is first, coverage for you and your dependents ends. Your dependents' coverage also ends if you die. However, they may be eligible for continued coverage under COBRA, as described below. Vision, Retiree Prescription Drug and Dental You and your spouse may be eligible for vision, retiree prescription drug and dental benefits when you retire. If you were employed by Food Fair/Pantry Pride Corporation, you are not entitled to dental benefits when you retire; however, you are entitled to vision and retiree prescription drug benefits. To qualify, you must have at least 10 years of full-time service with a contributing employer, be collecting a pension from a Food Industry Pension Fund, and be age 55 or older when you retire. If you are collecting a Social Security Disability Pension, you are eligible regardless of your age. Currently, your vision, retiree prescription drug and dental (if applicable) benefits continue for life. Coverage for your spouse ends at the time of your death unless you elected and are receiving a joint and survivor pension. In which case, vision, dental and retiree prescription continue for your spouse’s lifetime. Remember, your retiree benefits, as well as the cost to you, if any, for such benefits, may change from time to time, as determined by the Board of Trustees in its sole discretion. . |
