Keystone Point of Service Plan Benefits at a Glance

You will automatically be covered for Education, Child Care and Physical Well Being benefits. If you elect the Keystone Point of Service medical coverage, you must complete and return the Plan Selection Form within 60 days of the date you are notified that you are eligible for benefits.
