Convenience and Security Across the Country If you live or travel outside the Personal Choice Preferred Provider Network area, you will have access to the Blue Cross/Blue Shield PPO providers throughout the United States under the BlueCard PPO Program. More than 570,000 physicians and 4,400 hospitals across the country participate in the BlueCard PPO Program. On your identification card, you will notice a “PPO in a suitcase” logo. This logo is recognized by participating Blue Cross/Blue Shield PPO providers under the BlueCard PPO Program, assuring you preferred benefits and the same treatment and service throughout the United States Present your Personal Choice or Preferred Provider Organization ID card to any participating Blue Cross/Blue Shield PPO doctor or hospital, and you will receive preferred benefits. Participating Blue Cross/Blue Shield PPO providers submit the claim for you, and receive payment directly from their local Blue Cross/Blue Shield Plan; HOWEVER, YOU ARE RESPONSIBLE FOR OBTAINING ALL REQUIRED PRE-CERTIFICATIONS. You are only responsible for your usual preferred benefit level co-payment and/or coinsurance, where applicable. You still have the choice of using doctors and hospitals that do not participate in the BlueCard PPO Program; however, these services are paid at the non-preferred level and you are responsible for obtaining all required pre-authorizations. You may have to pay for medical expenses directly and submit the claim yourself. |
