Participating Facilities


    






RULES

Whether you use a participating facility or a non-participating facility, the Physical Well-Being Program is contingent upon the following terms:

Your eligibility for this benefit is based on the terms of your collective bargaining agreement.

You must remain continuously employed for the duration of your membership or participation in the program.

You must satisfy the following usage requirements:

Participating Facility- at least 10 usages for single memberships or 17 usages for family memberships per calendar quarter (Jan 1- Mar 31, Apr 1- Jun 30, Jul 1- Sep 30, Oct 1- Dec 31)

Non-Participating Facility- at least 20 usages in each calendar semester (Jan 1- Jun 30, Jul 1- Dec 31)

All claims for reimbursement must be submitted within 90 days of the end of the calendar year in which your membership or participation in a program occurred. Claims submitted after this period will not be reimbursed. 

If you know of any facility that may be interested in becoming a participating facility, please contact the Fund office with the name, location, and phone number. 

The benefits described herein are covered as of the distribution of this booklet and are subject to change. Any questions regarding this should be directed to the Fund office.
