USING THE PHYSICAL WELL-BEING PROGRAM


Participating Facility

When you join a participating facility, simply notify the facility that you are a participant of the UFCW Local 1776 and Participating Employers Health and Welfare Fund. You will be given an eligibility form which both you and the center will complete. The participating facility will then verify your eligibility with the Fund office. Once your eligibility is verified, you will then complete a quarterly utilization which will track your usage for each of the four calendar quarters. (See Rules for usage)

At many participating facilities, the Fund’s allowance will be considered payment in full. If, however, the participating facility requires that you make additional payments, you should be made aware of the specific amount and conditions of your co-payment at the time you join the facility. Call the Fund office to verify this amount.

Non-Participating Facility

When you join a non-participating facility, call the Fund office to request reimbursement information. In order to receive reimbursement, you must submit the following items to the Fund office for verification:

Completed reimbursement form indicating usage

Copy of itemized receipt of proof of payment

Copy of the membership agreement

If you sign up for an annual membership, both you and the center must complete the reimbursement form and submit these items twice each calendar year (June 30th and December 31st). You should call the Fund office to request this reimbursement form in June and December. If you join a program that is not utilized throughout the entire year (e.g. smoke cessation, weight loss, swim clubs, etc.), the reimbursement form should be requested upon completion of the program rather than in June or December.
